You should stamp this from your chamber of commerce.
An electronic copy can be found in Riyadh, Jeddah & Dammam chambers’ e-services
They have Mobily Authorization Form among their electronic services ready form.
For all other chambers you will need to download and fill the form, then stamp it from your chamber.
